At Zoe, we have a three part comprehensive process to ensuring the best outcomes for you and/or your child.

Assessments are important in order to get an accurate picture of you or your child’s communication and health abilities.  Without an assessment, it is difficult to know how we can help and plan treatment and therapy towards meeting your goals. Assessments are key for applying for funding, accessing supports for home, school and living and of course – diagnoses and establishing the things you or your child do well!

You may have had an assessment recently, or may be in need of one, for all of our clients – we follow our three part process to set you up for success.

  • Part 1:   Before we sit down with you, we will send you our Welcome Pack, with an ‘About Me’ link – this is a questionnaire all about you or your child. You know your child best, so taking time to fill this out informs us ahead of time of your history, needs and concerns. Attached to this is also information about our fees, policies, amazing Team and services! It’s one of those emails worth reading. 😉 We will also ask questions about what you are wanting to specifically know from the assessment so everybody is clear on the expectations.
  • Part 2:   The Initial Appointment – our favourite! We finally get to meet you! At this appointment, your Practitioner will ask more in-depth questions about you or your child as we review your questionnaire, to gain a comprehensive understanding of your needs, and how we can help across all environments – home, school, community, friendships. We will chat about your concerns and goals, we’ll do our observations and assessments to best establish ongoing supports. All assessments look different as they’re uniquely adapted for each individual to suit their temperament, age and stage of development. Typically we allow up to 90 minutes for this Initial Appointment. Depending on availability and/or abilities, our Practitioners may conduct this Initial over two separate 45 minute sessions. We will then complete your Treatment Management Plan, that is included in this process and is discussed in detail in Part 3.
  • Part 3:   This is the “Feedback & Goal Setting” appointment. Before we meet with you, we score, analyse and interpret the assessment results from part 2.  In part 3, we will review the assessment results together, discuss if therapy is required and work with you to set goals. You will be provided with a Treatment Management Plan at or soon after this appointment. Part 3 is best done as a parent/adult only session. This is because the information shared can be sensitive and not necessarily for “little ears”, and helps us to discuss our plan to have great outcomes! This appointment can be done online (similar to Zoom) to fit in with big schedules.

Treatment Management Plan:

The Zoe Allied Health Treatment Management Plan is a summary outlining the following information:

  • Things your child did well
  • Things your child found difficult
  • Therapy goals
  • Dosage of therapy
  • Service delivery (e.g. group therapy, 1:1 therapy etc)
  • Next steps required (e.g. onward referral to an audiologist etc)

We intentionally write the summary in a parent-friendly, easy-to-read format so that you can share the information with key people in the child’s life, should you wish.

Reports:

Our detailed reports contain a lot more information than the treatment management plan. A detailed assessment report is sometimes required, for example if your child is being referred onto the Paediatrician. Similarly, services such as school/kindergarten or NDIS require more detailed reports to secure funding.  This report will be more formally written and will contain scores if standardised/norm-referenced assessment tools have been used.

We typically have a turnaround time of 4 weeks to complete detailed assessment reports, and this is in addition to what is provided in our Initial Appointment process. Please let our team know upon booking if you will be requiring a Comprehensive Report after your Assessment and we can provide you with all the information.

Funding options:

We receive private, Medicare, NDIS Self and Plan Managed and DVA funded clients. See your GP about a Chronic Management Disease Management Plan to see if you qualify for Medicare rebates, or call us for more information. We look forward to seeing how we can support you.

Ongoing Support:

Research shows, high intensity has the best outcomes. We will work with you, your budget and availability to support you receiving the best ongoing options for you to target the goals highlighted through the assessment process. Appointments are encouraging, motivating and functional. We aim to help you understand why we are doing what we do and how to incorporate working towards your goals in an impactful and realistic way.

Online & Telehealth:

If online therapy is your preference, or if getting into the clinic is tricky, we will email you a link, by simply clicking on this link on either your computer, tablet, or phone, you will be able to access your appointment in real-time. You will be able to see, hear and talk with your therapist, just like a traditional, face to face appointment. Through this, your therapist will provide their usual intervention.

Intervention can be a six week process, or it can become a permanent part of life for a while, so we celebrate, and enjoy the wins along the way, taking stock of all the hard work the team (that’s you!) have put in to bring the ‘win’ into reality and take a moment to say ‘well done’!
